INSTRUCTIONS FOR FILLING OUT THIS FORM

1. Print or type the name of the entity which is the subject of the request.
2. Print or type the name of the person requesting expedited service (requesting party).
3. Print or type the address of the requesting party, including the name of the professional firm if applicable. Documentation will be mailed to this address.
4. A. Mail: Documentation will be mailed to the requesting party at 3:00 P.M. on the following business day.
B. Pick up: Documentation must be picked up before 3:00 P.M. on the following business day. Should the documentation not be picked up before 3:00 P.M., it will be placed in the outgoing mail and cannot be retrieved.
Expedited Fee is $25.00 PER TRANSACTION - For example: Review of a document and issuance of a confirmation filing is one transaction; a request for a certified copy is another transaction, etc.. EACH EXPEDITED SERVICE THAT IS REQUESTED ON THE REVERSE SIDE MUST BE PAID FOR WHEN THE REQUEST IS MADE. Specify the type of transaction to be expedited and, if requesting copies, identify the type of copy, the number of copies requested and the document to be copied by its type and, if applicable, its volume and page.
Please note that each time a document is reviewed on an expedited basis and rejected, the $25.00 expedited fee is consumed. Therefore, if documents are to be expedited upon re-submission, an additional $25.00 expedited fee must be resubmitted along with the documents.
EXPEDITED SERVICE means: The completion of the request within twenty-four hours of receipt of the request, except in those cases where the twenty-four hour period ends on a Saturday, Sunday, holiday, or day on which the Commercial Recording Division (CRD) is not open for business, in which case the twenty-four hour period is extended to appropriate hour of the next succeeding business day on which such CRD is open for business. (See Regulations of Connecticut Agencies 3-99a-2(b).
GENERAL INFORMATION
If you wish to order copies or other certificates on an expedited basis at the time you submit documents for filing, you must submit an additional expedited fee for each transaction.
Long form certificates of existence cannot be issued on an expedited basis.
If you wish to expedite the review of documents and have copies or other certificates issued in the ordinary course of work, please state this clearly in a cover letter accompanying the filing.
If you mail documents or requests to this office and wish to utilize expedited service, either include an expedited request form or clearly specify your request in a cover letter.
In the absence of specific instructions, this office will allocate expedited fees submitted to the review of documents first and to the processing of certifications or copies second.
